Each student must have the OHSAA PPE (School Year ’26-’27) signed and
completed by a parent or guardian.
Students entering 6th-12th grade may participate.
Please wear athletic attire and your contacts/glasses if you have them.

Cost: $20.00 Cash or Check ($10.00 of the payment comes back to La Salle Athletic Training)
